How to Submit a Superbill
A superbill is a detailed receipt from your therapist that you can submit to your insurance company to request reimbursement for therapy sessions. Here’s how to do it:
Step One
Request Your Superbill
- Ask your therapist for a superbill after your session(s).
- A superbill typically includes:
- Therapist’s name, license, and NPI number
- Client information
- Dates of service
- CPT codes (the type of therapy session)
- Diagnosis codes
Step Two
Contact Your Insurance Provider
- Call your insurance company or check their website to learn their out-of-network reimbursement policy.
- Ask if they require a claim form to be submitted along with your superbill.
- Fill out any required claim forms with accurate personal and insurance details.
- Attach the superbill provided by your therapist.
Step Three
Submit Your Claim and Receive Reimbursement
- Send the claim form and superbill to your insurance company.
- Submission options may include:
- Email or online portal (if accepted)
- If approved, your insurance company will issue a payment to you based on your out-of-network coverage.
